By checking this box and signing your name below, you hereby waive and release Team Houston Baseball and its contracted facilities from all claims, damages, demands, rights of action and liabilities, present or future, known or unknown, anticipated or unanticipated, resulting from or arising out of the athlete's participation in Team Houston Baseball activities or programs, including, without limitation, those arising from our negligence or that of any other member of the Team Houston Baseball Corporation or its contracted facilities.

Spring, 2025 season will consist of 8 tournaments, 2 practices/week, and a World Series for each youth team ages 10U-14U. Practices will begin February 11th and go through mid-July. Please take note of the payment schedule below.

You also acknowledge that you understand if your son leaves the organization before the end of the season, your deposited payment(s) are non-refundable unless your son has a significant injury that is verified by a physician.
